The inclusion of transition services in Article 89 of the Laws of New York
State, as amended by Chapter 699 of the Laws of 1993, amended in January 2000,
affirms the Individuals with Disabilities Act (IDEA). Educators, parents and students
are now provided with the opportunity and methods to prepare secondary students with
disabilities for living, learning, working and participating much more successfully in
the community as adults.
The Individual Education Plan (IEP) planning process enables parents,
students, adult agency representatives, and other members of the school and community to
work together in creative ways to develop and carry out services that help students make
successful transitions to adult life. This is a strength based process and focuses on the
student's aspirations for the future. The Long Term Adult Outcome Statement, which is required on the student's IEP beginning at age 14 and updated annually, should acknowledge the students
preferences and interests. The Long Term Adult Outcome statement should be the driving
force of the student's educational plan. This statement is the basis for determining needed
activities that will promote student movement from school to post-school outcomes.
The Long Island Region Transition Coordination Site is one of seven sites
around New York State. The Long Island site in collaboration with the State
Education Department and the office of Vocational and Educational Services for Individuals with
Disabilities (VESID), provides individualized technical assistance to all school districts,
parents, students and adult service agencies in Nassau and Suffolk County in the areas of transition planning. All Long Island districts have access to the areas of transition planning. All Long Island districts have access to the information necessary to accomplish this goal as a result of this Transition Grant.
Transition Site activities include workshops, workdays, job coach training,
implementing and coordinating the Post School Indicators Survey (PSI), and individualized
technical assistance. We also provide resources to students, parents, adult service agencies, and
school district staff. The site gives training on the development of individualized education plans (IEPs) that must include transition. Other trainings include curriculum development, and
training on the Career Development and Occupational Studies standard (CDOS). The site has a
