Students in the Person Centered Transition Planning Program are selected from three area school districts and three learning centers. The students involved in the Person Centered Transition grant are developmentally disabled. One student is also medically fragile. They have been chosen by their teachers to participate and are within the last two years of their educational program.
The districts represented have indicated a willingness to change their current traditional transition planning methods to one that incorporates the person centered philosophy. Ongoing parent, family and teacher involvement will be critical to the ultimate success of this system change initiative.
