The Long Island Person Centered Planning project has as its goal to provide non-traditional and varied opportunities for students with developmental disabilities to live, learn and work in their community. They will be able to choose what they wish to do with their lives and find appropriate supports to assist them.
We are also providing computers and internet access to all the families participating in the grant. This site will enable the families and professionals to have an open and ongoing dialogue about transition planning, connect to community resources and network with each other about common issues.
Person Centered Transition Planning (PCTP) is a straightforward name that describes a process that allows people to plan purposeful and meaningful change in their lives by centering those plans specifically on the person.
The plan consists of tools that have been developed to help one person or a family. The tools are designed to serve the hopes, dreams and vision of the person or family on which the plan is focused.
In many ways, PCP is simply good, values-driven planning. Good planning starts with the realization that every person has an individual, idiosyncratic, possibly eccentric, view of what matters in life. Good planning involves those who know the person best. It engages a broad array of resources and is a creative process at heart. |
